Every published version contains chromium and gymnema, which lower blood sugar. Combined with insulin, a sulfonylurea or metformin the effect is additive. Never reduce or stop a prescribed medication because a supplement seems to be working; stopping one risks diabetic ketoacidosis. Specific cautions, depending on which formula your bottle carries

• Grapefruit extract (22-ingredient version): raises blood levels of statins, calcium-channel blockers, some immunosuppressants and sedatives. Ask a pharmacist.
• Licorice compound (22-ingredient version): raises blood pressure and lowers potassium with continued use. Ask first if you have hypertension, heart or kidney disease, or take diuretics.
• Caffeine (green tea on two lists, guarana on one): undeclared. Take in the morning; avoid if pregnant, caffeine-sensitive, or with a heart rhythm problem.
• Berberine (eight-ingredient version): interacts with metformin, cyclosporine and many CYP-cleared drugs; unsafe in pregnancy and breastfeeding.
• Kidneys: diabetes is the leading cause of kidney disease; anyone with reduced kidney function should be cautious with concentrated botanical blends.
• Adults only. Not for anyone under 18, pregnant or nursing. Keep out of reach of children.

Symptoms that need a doctor

Persistently high readings, unusual thirst, frequent urination, unexplained weight loss, blurred vision, or tingling in the feet need clinical assessment, not a supplement. A “nerve guide” bundled with a purchase is not a response to tingling; a doctor is.
